Decision Making and Consent Policy 

A policy guide to inform service providers and staff who need to obtain consent from a person with a disability or a guardian.

Health Care Policy

A policy guide for staff to support people with a disability to be as healthy as possible, and to have a Health Care Plan developed and implemented to meet their individual needs.

Note: Attachment 11 is the Comprehensive Health Assessment Program (CHAP). Due to the licensing arrangement between ADHC and the owner of the CHAP it cannot be put on our website. Service providers can obtain a copy of the CHAP from ADHC's Service Portal.

Medicare item numbers for comprehensive health assessments of people with intellectual disability

The Australian Government has introduced four new Medicare items for general practitioners (GPs) to assess the health of people with intellectual disability and other 'at risk' groups. Visit the Medicare Benefits Schedule website for more information.

Nutrition and Swallowing Policy 

This is an overarching policy for supported accommodation and centre based respite care services funded or directly provided by ADHC. This policy establishes the principles of good nutrition support for people with a disability and outlines standard and minimum practice requirements and includes the Nutrition and Swallowing Checklist. The Checklist is a screening tool to be used for people with nutrition and swallowing problems.

This manual aims to assist service providers to provide food and nutritional care, which meets the needs of their clients with a disability. Specifically, the manual provides information to be used with the Nutrition and Swallowing Risk checklist. The manual also provides information on a range of topics that could be useful in helping people to achieve good health through good nutrition.

Palliative Care Policy 

This document sets out policy for ADHC funded or operated accommodation support services on requirements for the development, implementation and review of Palliative Care Plans. Funded services may develop their own procedures or adapt and use operational procedures outlined in Part 3.
