Leaving Care Program
This program supports young people with a disability to transition from the parental responsibility of the Minister for Community Services and to live as independently as possible.
It does this by providing transition planning, case management and access to:
- Support to make lifestyle choices;
- Training for independence;
- Economic independence;
- Community connection;
- Accommodation; and
- Mentoring service.
Case management
Each young person leaving care is the driver of their planning process, supported by Community Services (prior to transition), ADHC and their Leaving Care Case Coordinator (appointed under this program).
