Delivering HACC services

The Home and Community Care (HACC) Program is a joint Australian and state/territory government program that provides support to frail aged people, younger people with a disability and their carers. The program helps clients remain at home and prevent inappropriate or premature admission to residential care.

HACC service type guidelines

The National Program Guidelines for the HACC Program are mandatory for service providers who are funded to provide HACC Services. Additional Program Guidelines have been developed for HACC-funded services in NSW. A good practice guide for service providers delivering case management is also available.

Service type descriptions

Each service type is described relative to the service to be provided. The service type is identified in, and forms part of, the service provider contract.

HACC policy

HACC policy documents describe the parameters of the HACC Program and its operation and give guidance for those involved with the program.

Service provider tools

All HACC service providers must comply with the Community Care Common Standards. The Common Standards for community Care have replaced the national HACC Standards commencing from 1 March 2011.

Funding

The HACC program is a jointly funded Australian and state/territory government program, established under the Home and Community Care Act 1985.

Each financial year, the Ministers representing the NSW and Australian governments approve the HACC Annual Supplement, specifying the planned service outputs and funding allocations in each Local Planning Area (LPA) and statewide and capital projects.

See funding and grants for details on how funding is allocated for delivery of services and Tenders NSW for information on current and past tenders.

HACC Resource Allocation Formula

The distribution of total HACC Program funding across Local Planning Areas (LPAs) is based on a Resource Allocation Formula (RAF).

HACC planning

Comprehensive planning is undertaken for the delivery of the NSW HACC Program.

The Better Practice Project

The NSW Government is encouraging innovative and better practice approaches within HACC in NSW through The Better Practice Project.
