Home Modification Reforms

From 1 July 2012, Ageing, Disability and Home Care (ADHC) introduced a series of reforms to community care home modification services. Following the introduction of the reforms on 1 July 2012, further changes were made effective on 23 July 2012. Referrals to home modification services in the Hunter and Central Coast local planning areas continue through the Community Care Access Point. All other areas have returned to previous referral pathways while the reforms continue to develop.

Phase 2 Reforms

ADHC's Community Care Directorate will continue work on the planning and implementation of the Phase 2 reforms to community care home modification services. You will be kept informed of opportunities for consultation and input.
