Equipment and modifications fund
Equipment and Modifications Funding is available to individuals accessing Life Skills and Employment Programs, including:
- Community Participation
- Transition To Work
- Life Choices
- Active Ageing.
Service providers apply for Equipment and Modifications funding on behalf of an individual currently accessing their service and for individuals who use the intermediary funding arrangement.
The Equipment and Modifications Fund for Life Skills and Employment Programs can be used to fund:
- Assistive equipment and technology, including any item or piece of equipment that is used to increase, maintain or improve the individual’s functional capabilities
- Building Modifications to a day program centre, including structural changes to improve safety or accessibility for the individual(s).
- Other items, including modifications to motor vehicles, may be considered on a case-by-case basis.
The consent of the individual and/or their family/carer/guardian/advocate must be obtained prior to their information being submitted to ADHC.
The application must be supported by a report from a relevant professional, for example a speech pathologist or an occupational therapist. The report should include an overview of the assessment completed, methods used to assess the needs of the individual and the rationale for the recommendations.
Applications should be submitted to the local FACS district office.
